Job Summary

This position serves on the Scott County Health and Human Services, Disability Services department. It is a full-time position that is primarily based out of the Scott County Government Center, with the potential for a hybrid work arrangement. Hours are generally M-F, 8-4:30, but may vary based on service needs. This position serves as the liaison between the Contracted Case Management (CCM) agencies and the Scott County internal Supervisors within the Disability Services unit. Tasks involve coordinating the flow of information to and from our external partners utilizing technology platforms to ensure secure and reliable exchanges of information. The position connects CCM partners to resources available outside the Disability Services unit to maximize supports available to program participants. The Lead participates in Metro County monthly meetings to gather updates about programs and policies related to services authorized through the Disability Services unit. The Programs Lead is also responsible to implement and monitor quality assurance measures, provide technical assistance, develop trainings as needed, and be a warm, welcoming, knowledgeable, reliable person for our growing Contracted Case Management programs."

Job Description

ESSENTIAL DUTIES

  • Develops and maintains professional relationships with contracted case management agencies, the Minnesota Department of Human Services, and a wide network of disability service providers.
  • Serves as primarily liaison to contracted agencies.
  • Monitors and addresses compliance issues related to CBSM services and supports.
  • Provides oversight and approval of program and individual level budgets for Waivered and Non-waivered Services.
  • Provides CBSM service expertise to billing staff in order to authorize revenue and expenses.
  • Advises contracted case management staff in the development and implementation of service plans.
  • Advises Social Service Supervisors in the development and implementation of service goals and objectives for contracted work.
  • Advises management on contract terms and rate setting methodology for contracted providers.
  • Gathers, analyzes, and evaluates data ensuring positive outcomes.
  • Advises internal review committee for Rule 185 eligibility and monitors contracted case assignments.
  • Maintains Contracted Case Management Provider Manual.
  • Participates in metro contracted case management and other collaboratives.
  • Attends and participates in scheduled staff meetings. Leads contracted staff meetings.
  • Provides education, consultation, and training for other internal and external agencies.
  • Demonstrates working knowledge of federal and state laws and regulations, as well as professional standards of practice, related to case management and community support programming.
  • Works the days and hours necessary to perform all assigned responsibilities and tasks. Must be available (especially during regular business hours or shifts) to communicate with other employees, supervisors, customers, vendors, and any other person or organization with whom interaction is required to accomplish work and employer goals.
  • Performs other related duties as required or assigned.

MINIMUM QUALIFICATIONS


Requires equivalency of a Bachelor’s Degree in Social Work or related field and 5 years experience in the provision of state waivered and non-waivered services. A valid driver's license and a reliable form of transportation for the performance of work responsibilities are required. Preference given for background or experience working collaboratively with contracted case management partners, previous experience working with county Social Service Agency and/or in a social services setting, knowledge of and experience with person-centered strategies, and demonstrated ability to build relationships and work effective in cross-cultural settings. Ability to speak a second language (Spanish, Somali, American Sign Language, Russian), previous experience or working knowledge of SSIS, and qualified as QDDP are also desired.

SUPERVISORY CONTROLS


The employee is under general supervision of the Social Work Supervisor. Work is performed independently according to established procedures and guidelines. The supervisor may occasionally review completed work to ensure adherence to departmental policies and procedures. Incumbent provides work direction to but contracted provider supervisors and managers. They may also direct line staff within the organization to ensure compliance with the Scott County contracts, statutes, and state policy.

WORK ENVIRONMENT AND PHYSICAL DEMANDS


Typical characteristics of the regular, ongoing work environment of this position requires inside work, typically in an office setting.

Physical activities include sedentary to light work: Reaching, sitting, standing, walking, pushing, lifting, pulling, fingering, grasping, feeling, stooping, talking, hearing, seeing, and repetitive motions. Lifts and/or navigates up to 35 pounds on a routine basis in handling files and equipment necessary for performing the essential duties of the job.
