Job Summary
Under the general direction of the Public Health Supervisor, employee performs responsible administrative duties including the following: preparing documents, organizing, and maintaining case records, providing the public with requested information and referrals, and performing related work as required. The employee has the authority to carry out their duties in a manner consistent with professional judgment and within the parameters of state statutes, Department of Human Services rules, and Scott County Human Services standard operating procedures.
Job Description
ESSENTIAL DUTIES
- Provides program outreach activities that increase community awareness and promote utilization of C&TC screening services through telephone calls, letters, presentations, face-to-face outreach activities. Works to reduce barriers to healthcare access for C&TC enrollees. Designs, develops, and utilizes visual teaching for C&TC outreach at community sites. Documents program activity utilizing computer programs supporting C&TC outreach.
- Assists with planning, developing, and implementing work methods and procedures to support effective delivery of program activities in the office and at clinic and community sites.
- Accesses and navigates several state and county computer systems and databases for work tasks and documentation.
- Writes, edits, and sends out bulk email messages to clients or providers for efficient communication.
- Assists with program communications to departmental staff, community partners, schools, and the public.
- Educates clients on program benefits and facilitates access to health services and other community resources.
- Creates and produces documents and presentation materials utilizing department resources.
- Schedules interpreters for other languages as needed to communicate with non-English speaking or hearing-impaired clients.
- Researches health promotion materials, monitors inventory, and initiates ordering process for program materials and supplies.
- Designs, develops, and utilizes visual teaching for C&TC outreach at community sites.
- Provide parents with guidance, education, information, and resources that help them support their children through the different stages of development. - Performs administrative duties including the following: preparing documents, organizing and maintaining case records, providing the public with requested information and referrals.
- Works the hours and/or shifts assigned and begins and ends work on time. Provides back-up for other areas of operation as required, requested, and/or assigned.
- Performs other related duties as required or assigned. Includes but is not limited to emergency preparedness and response, including fulfilling roles in an incident command structure as needed.
MINIMUM QUALIFICATIONS
Requires equivalency of high school graduation and four years of Human Services experience in a direct- or support-service function. A Bachelor's Degree in a related field may substitute for the experience. Preference given for experience, especially MMIS. Bilingual and the ability to translate and interpret are desired. The incumbent shall provide a valid driver's license and a reliable means of transportation for the performance of work responsibilities.
SUPERVISORY CONTROLS
The employee is under general supervision from the Public Health Supervisor. The employee works with considerable independence completing routine assignments and special projects. Work is reviewed for accuracy and consistency. Annual reviews and ongoing supervision by the Public Health Director will assist the employee in job performance standards.

WORK ENVIRONMENT AND PHYSICAL DEMANDS
Typical characteristics of the regular, ongoing work environment of this position requires inside work, typically in an office setting. May transport clients when needed.
Physical activities include sedentary to light work: Reaching, sitting, standing, walking, pushing, lifting, pulling, fingering, grasping, feeling, stooping, talking, hearing, seeing, and repetitive motions. Lifts and/or navigates up to 35 pounds on a routine basis in handling files and equipment necessary for performing the essential duties of the job.
